More complicated operation is the elimination of a sharp divergence of the plates of the bifurcated quadrangular cartilage; the plates have to be cut off from the point of their connection with the coulter along the entire length in order to be able to bring them closer together to the center line. Instead of a swallow skin incision, it is more convenient to make a skin incision along the midline of the nasal dorsum, since it gives wider access to the quadrangular cartilage.

We observed a patient with a split nose, who had a complete splitting of the quadrangular cartilage into two plates. A blind sac lined with mucous membrane was laid between the plates. From the cavity of the sac there was a fistula that opened into a transitional fold upper lip between the two bridles. To eliminate this deformity, a skin incision was made transversely over the wings of the nose and vertically between the wing cartilages. The bag was husked, the plates of the split quadrangular cartilage, after cutting them off at the point of attachment to the vomer, were sutured. The nasal bones are brought together and the skin between the wings is partially excised (Fig. 231, a, b and Fig. 232).

Another patient had only one half of the nose with one nasal passage. The other half was completely absent; it was restored with a double-bent forehead flap.
